How to Import a Spreadsheet to Create a New Lov in Application Builder?

I’m struggling to import a spreadsheet to create a new LOV in my data model in the Application Builder. I see a red error dialog pop up and it blocks me from import. But there is no error message on the pop up or in the PDE log/Error View. Where can I track down what’s wrong with this LOV import?

The Application Builder only imports the first tab, named 'Import'. In the tab, 'Import', there are several rows after the last code WD99 which are empty rows but have formatting. These formatted rows (containing shading and borders) is considered data even if it is empty of values.
Delete these rows and the spreadsheet imports without any problems.
